I, entirely upon my own initiative and risk and responsibility, agree to participate in physical activities at ALPHA OMEGA MARTIAL ARTS, ALPHA OMEGA JIU-JITSU, and LAREDO KARATE CENTER. I am at least 18 years of age OR If I am under 18 years of age, a parent or guardian consents.

In consideration for participation in Martial Arts training (including but not limited to Jiu-Jitsu, Karate, Boxing, Kickboxing, etc.), exercise training, and other valuable consideration, I hereby RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO HOLD ALPHA OMEGA MARTIAL ARTS, their officers, servants, agents, and employees, ALPHA OMEGA JIU-JITSU, L.L.C., their officers, servants, agents, and employees, and LAREDO KARATE CENTER, their officers, servants, agents, and employees (hereinafter referred to as RELEASEE) from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me, WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEE, or otherwise, while participating in such activity, or while in, on or upon the premises where the activity is being conducted or in transportation to and from said premises.

I understand that Martial Arts and rigorous physical exercise can cause serious injury or even death. I will be learning to immobilize an opponent. I freely choose to participate in class and acknowledge my risk. To the best of my knowledge, I can fully participate in this activity. I am fully aware of common risks and hazards connected with the activity as follows, including but not limited to: bruises, scratches, bloody lips/noses, torn ligaments, broken bones, dislocation of joints, contact dermatitis, and other contact related skin conditions, and I hereby elect to voluntarily participate in said activity, and to enter the above-named premises and engage in such activity knowing that the activity may be hazardous to me and my property. I VOLUNTARILY ASSUME FULL RESPONSIBILITY FOR ANY RISKS OF LOSS, PROPERTY DAMAGE OR PERSONAL INJURY, INCLUDING DEATH, that may be sustained by me, or any loss or damage to property owned by me, as a result of being engaged in such an activity, WHETHER CAUSED BY THE NEGLIGENCE OF RELEASEE or otherwise.

I further agree to become familiar with the rules and regulations of the school and program concerning student conduct and not to violate said rules of any directive or instruction made by the person or persons in charge of said activity and that I will further assume the complete risk of any activity done in violation of any rule or directive or instruction. 

I acknowledge that the Martial Arts taught at Alpha Omega Martial Arts, Alpha Omega Jiu-Jitsu, and Laredo Karate Center is for self-defense, self-cultivation, and self-discipline purposes only and is not meant to be used to instigate conflict.

Therefore, I do hereby for myself, my heirs, executors, and administrators release and hold harmless Alpha Omega Martial Arts its board members, its employees, its agents; Alpha Omega Jiu Jitsu, its board members, its employees, its agents, the head instructor Michael Esquivel II; Laredo Karate Center, its board members, its employees, its agents; and any guest instructors from any and all liability, negligence, claims, demands, actions or causes of actions, or caused of actions resulting from any participation in this sport, activity, class, and associated activity.

This Waiver of Liability and Hold Harmless Agreement shall be construed in accordance with the laws of the State of Texas.
